BMS to Offer Immunization Clinic at Spring Parent Teacher Conferences

IMMUNIZATION CLINIC AT BMS DURING PARENT TEACHER CONFERENCES


**ATTENTION PARENTS**


WHO: This clinic is being provided by Regional West Medical Center Community Health Immunization Clinic for any student needing vaccinations.


WHAT: In preparation for 7th grade, your child will be required to have record of certain immunizations. These are to be completed prior to the first day of 7th grade! To help get your student ready for 7th grade, an immunization clinic will be held at BMS while you attend Parent Teacher Conference. We will have the vaccines recommended by the CDC for this age group available (TDAP is required, HPV, Meningococcal are recommended). Please bring your current immunization record if possible.


ANY BMS STUDENT NEEDING VACCINATIONS IS WELCOME REGARDLESS OF WHAT GRADE THEY ARE IN


WHERE: Bluffs Middle School Health Office


WHEN: Thursday, March 21 - 3-7:00 PM


COST: Bring your Health Insurance or Medicaid card. If you do not have health coverage at this time, then the vaccination will be covered through the CDC Vaccine for Children Program at no additional cost to you.


**SCHEDULE AN APPOINTMENT **


Check your student’s immunization record, if he/she needs vaccinations; contact Aimee Wheeler at #225-0726(CALL OR TEXT) or awheeler@sbps.net to schedule an appointment.


Parent or Guardian must accompany the student to the appointment. The appointment will take approximately 20-30 minutes.
